Out of State Trained Guide Form - NOTE:  This is only acceptable for guides who received adequate training outside of Colorado.  Guides trained in Colorado in 1985 or later must have a completed Minimum Guide Qualification form.

Outfitters Boat Accident Report - Complete & Print to Mail OR submit digitally.  NOTE:  any accidents that occurred within the Arkansas Headwaters Recreation Area will be forwarded to them immediately.
